General Description:

Team members needed for an innovative case management agency which works alongside people of all ages with long term care needs. Service Coordinators assist people to design and receive services that they need within a highly regulatory environment. Service Coordinators build meaningful relationships with people, families and community providers while completing essential responsibilities around service/care planning and monitoring activities. Must be adaptable to change and have the ability to produce quality work in a variety of environments and locations against multiple deadlines. This job requires comfort with technology, adaptability to people and systems, extensive driving responsibilities and a commitment for helping those in need.

Education and Experience Requirements:

Minimum: A bachelor's level degree of education, five (5) years of experience in the field of human services, or some combination of education and experience appropriate to the requirements of the position.(experience must be specifically documented and verifiable on your application.) Excellent written and verbal communication skills. Strong advocacy skills. Preferred: Graduation from an accredited college or university with a bachelor’s degree in Sociology, Psychology, Education, Human Services or other closely related field. At least one year experience in a case management environment. Experience working with families and community resources; knowledge of family dynamics and resource development. Working knowledge of Medicaid Waiver programs and requirements; experience working with people in a community, employment or recreational setting.

Material and Equipment Directly Used:

Computers, Printers, Copy, and Fax Machines, Personal Vehicle, Surface Pro/tablets, Cell phones, etc.

Working Environment/Physical Activities:

Office and Community Sites. Moderate physical activity required. Frequent sitting and standing. A working vehicle, valid CO driver’s license (unless active Military family), and minimum insurance levels ($100,000/$300,000/$100,000 liability coverage) are required. Salary Range: $15.00-$19.00 an hour (depending on experience)

Mileage Reimbursement: IRS rates
